The Healthy Steps Specialist coordinates medical care and provides psychotherapy and screenings for pediatric patients aged 0 to 5. They collaborate with medical professionals to implement early childhood development programs and connect families with community resources.
Candidates must hold a Master's degree in social work and a valid NYS LMSW license. Experience in early childhood growth, parent-child relational health, and infant mental health is required.

The Healthy Steps Specialist is responsible for comprehensive coordination of medical care (linkage to supportive services and retention in care) to improve health outcomes for patients of Evergreen Health Primary and Specialty Care. They will provide interventions, screenings,psychotherapyand follow up for families and pediatric patients from age 0 to 5. The Healthy Steps specialist will build strong relationships with families and providers to increase the efficiency of the medical system, and support team-based comprehensive care.
Qualified candidate will have a Master’s degree in social work, NYS LMSW license, with knowledge and experience in early childhood growth and development, parent-child relational health, infant and early childhood mental health, and/or family systems; excellent written and verbal communication skills; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential

This position is posted with a salary range. The actual rate of pay offered will be based on a variety of individualized factors, including but not limited to a candidate’s skills, relevant experience, educational background, internal equity, and market considerations.
