The Healthy Steps Specialist is responsible for comprehensive coordination of medical care (linkage to supportive services and retention in care) to improve health outcomes for patients of Evergreen Health Primary and Specialty Care. They will provide interventions, screenings,psychotherapyand follow up for families and pediatric patients from age 0 to 5. The Healthy Steps specialist will build strong relationships with families and providers to increase the efficiency of the medical system, and support team-based comprehensive care.

As part of the essential functions of this role

  • The

  • Specialist:
  • Conducts team-based well child visits with parents and their babies and young children with the physician/pediatric nurse practitioner
  • Provides consultations with families with babies and young children regarding developmental and behavioral concerns such as sleep, discipline, picky eating, etc.
  • Collaborates with Healthy Steps team to implement all core components of the program, including universal child development and SDOH screening, making positive parenting and early learning guidance information readily available to all practice staff, and adjusting workflows for optimal efficiency.
  • Provides consultation to medical professionals and all practice staff re: early childhood development, infant mental health, and trauma-informed practice.
  • Provides referrals and tracks follow-up, as appropriate, to help families make connections to key resources within the community.

Qualified candidate will have a Master’s degree in social work, NYS LMSW license, with knowledge and experience in early childhood growth and development, parent-child relational health, infant and early childhood mental health, and/or family systems; excellent written and verbal communication skills; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential
