The Care Coordinator assists all patients through the system by acting as the administrative assistant, patient advocate and navigator with emphasis on engagement of the individuals and families.
Essential Duties $ Responsibilities
- Works closely with all school administration, school social workers, school nurses and other support staff to coordinate care. Communicates with a parents/student via phone or in- person to coordinate care and obtain completed enrollment forms/consent. Performs data collection pertaining to SBHC and other administrative functions Maintains registration lists, updates spreadsheets and demographic information, obtains consents and Ferpa related releases. Coordinates with IC primary care for referral and other patient/administrative needs In charge of Scheduling and check in process of new patients and scheduling follow up appointments for existing patients. Collaborates with all inter-disciplinaries for continuity of medical/behavioral careAnswers phone calls, creates electronic medical record for all new patients, scans all information into primary care and behavioral health medical record. *All agency staff are required to attend all mandatory department/agency meetings and trainings
- Schedule:
Monday - Friday, 8 AM - 4: 30 PM (12 month position)
Requirements
