DUTIES/RESPONSIBILITIES: Engage in Intensive-In-Home, Mental Health Skill Building, Community Stabilization, Mobile Crisis Response, and/or Psychosocial Rehabilitation Services. Makes contact with individual/family and referral source within 24 hours of accepting referral. Engage individuals in individual and/or family sessions while providing psychoeducation, behavioral support, and therapeutic interventions. Utilize verbal de-escalation techniques and strategies to assist the individual with maintaining self-control. Educate the individual on and assist them with utilizing coping skills to manage behaviors and emotions. During IIH Services, engage in services specifically designed to improve family dynamics, provide modeling, and the clinically necessary interventions that increase functional and therapeutic interpersonal relations between family members in the home. Promote psychoeducational benefits in the home setting to assist the individual and family with avoiding an out-of-home placement or assist with transition home from an out-of-home placement. During MHSS, engage the individual in goal directed training in the following areas: functional skills and appropriate behavior related to the individual’s health and safety; activities of daily living, and use of community resources; assistance with medication management; and monitoring health, nutrition, and physical condition. During MCR services, provide direct therapeutic care to children, youth and adults who are in psychiatric crisis. Work to stabilize behaviors to assist the client with avoiding out of home placement. Provide behavioral support to clients experiencing a mental health crisis. For PSR services, be present and maintain ratio of group participants throughout program hours of operations. Provide education and skills building in independent living, prevocational, academic, social, and communication skills to participants in group setting. Assess, evaluate and monitor the safety of individuals and others at all times. Implements individual behavioral modification and safety plans when applicable. Develop person-centered, individualized treatment plans based on recommended goals identified in the individual’s assessment. Complete all required documentation in a timely manner which includes but not limited to, the tracking of all individualized behaviors, individual service plans, monthly reports, quarterly reports, discharge planning, progress notes and all related documentation in compliance with DMAS, Licensure and Dominion Care standards. Engage in Care Coordination with other staff and community service providers to ensure the appropriateness of treatment modalities and services being provided. Attends supervision and staff meetings to address the individual’s needs/progress and any administrative needs. Provide 24-hour emergency response as needed. Attends all meetings related to case management needs of individuals (IAP’s, Med Evals, FAPT, psychological testing, etc.). Maintains contact with program leadership and referral agencies on a regular basis to discuss updates, progress, concerns, or provide feedback. Develops and maintains a network of supportive community referrals for individuals. Refers and links individuals to community resources. Establishes working relationships with representatives of other agencies who work with and provide services to individuals. Ensures the appropriate level of service is provided to each individual. Maintains compliance with all Licensure and Medicaid Standards. Adhere to all Dominion Care policies and procedures and complete other tasks which may be necessary for the successful management of the program.
