Job description

SUMMARY

Works in team collaborative setting to provide child welfare placement and support services for children and families. Meets frequently with children and families in their preferred setting to include school, the family residence and in the community. Completes required documentation.

Duties

1. Develops and maintains supportive relationships with children, families, and significant support systems by meeting with them frequently in their preferred setting.

2. Provides support services to assist in the day-to-day functioning of the Foster Care programs and foster parent training and licensure.

3. Assists in navigating multiple systems of care by providing information to the Foster Care programs concerning community resources and linkages to other services.

4. Assists children and families in securing appointments with informal and formal services and agencies, as needed.

5. Arranges and coordinates respite and HCTC placements, as well as, billing for the HCTC program

6. Maintains current documentation for foster care program with the child and family and the services received/provided.

7. An individual may qualify for the position if the individual is able to “perform safely” the essential functions of the position with or without a reasonable accommodation. “Perform safely” means to perform without causing a direct threat to the health or safety of the individual or others that cannot be eliminated by reasonable accommodation, as defined by 42 U.S.C. #12111(b) and the EEOC’s regulations, 29 C.F.R #1630.2(r).

8. Performs other duties as assigned.

Requirements: Training and Experience

High school diploma or equivalent. Must be able to obtain full fingerprint certification.

Knowledge, Skills, Abilities

1. Knowledge of community social services

2. Ability to function as a member of a Family Centered Team in a Managed Care environment

3. Ability to maintain good consumer relationships

4. Excellent verbal and written communication skills

5. Ability to identify priorities and stay organized

6. Ability to work with a diverse population of consumers and employees from different racial, ethnic, economic, and social backgrounds.

7. Ability to operate a motor vehicle

8. Ability to operate personal computer keyboard and telephonic communications equipment

9. Ability to read the screen of a personal computer monitor

10. Ability to read handwritten and printed documents and records

11. Ability to perform analysis of moderate complexity

12. Ability to move around an office environment

13. Ability to communicate ideas of moderate complexity in writing

Other Potential Requirements: Bilingual (Spanish/English) preferred

Availability to work evening and weekend hours

Possession of first aid and CPR certification.
