The Community Health Improvement (CHI) Wellness Coordinator is responsible for coordinating and integrating various wellness programs and services within Community Partnership Schools (CPS) in collaboration with UF Health St. Johns Care Connect that align with the Community Health Needs Assessment. This role ensures the delivery of proactive behavioral health, wellness support, health education, and social support for students and families in the community through referrals. This includes organizing events with healthcare initiatives and collaborating with the mobile outreach clinic team to increase access to care and contribute to health equity. The Wellness Coordinator also oversees the maintenance and functionality of the telehealth kiosk, providing access to health services for community members, students, and school staff.
Responsibilities
Manage and track the school healthcare programs' referral network and processes, ensuring efficient service delivery.
Monitor and follow up on all referrals made within and outside the school to ensure continuity of care.
Act as the primary contact between UF Health St Johns Care Connect, CPS administration, school nurses, medical clinics, behavioral health departments, MOC, and other health-related agencies providing services.
Oversee the operation and upkeep of the telehealth kiosk to ensure accessibility for community members, students, and staff.
Provide support and troubleshooting assistance as necessary.
Schedule appointments for patients that need a telehealth visit.
Establish and grow referral network.
Develop and maintain linkages with external agencies to support comprehensive wellness services.
Plan and implement wellness activities and education to engage families in on-campus and community services.
Coordinate health and wellness events for students, staff, and the community including tabling, health education, clinics, and other healthcare initiatives.
Participate in trainings and site visits related to the Community Partnership Schools development or Community Health Improvement department.
Submit and track all referrals, including health, dental, vision and mental health referrals.
Participate in Intervention Team meetings to provide information on behavioral interventions and relevant referrals as needed.
Document in electronic referral system and maintain records for accurate data collection.
Maintain confidentiality and adhere to HIPAA regulations.
All other duties as assigned by the department and leadership.
Qualifications
Required Education
Associate's Degree.
Specific course work: Social services, Human Services, Public health, Psychology or related area of study.
