The Case Manager conducts home visits to assess family needs and provides prevention services focused on child development for expectant and new parents. They are also responsible for community outreach to increase program enrollment and linking families to essential community resources.
A high school diploma and at least six months of experience working with children and families are required. Candidates must be bilingual in Spanish, possess a valid NYS Driver's License, and have knowledge of infant and child development.

Family Service League is seeking a full-time Bilingual Case Manager for the Healthy Families Suffolk program. The Bilingual Case Manager reviews referrals of prospective participants to determine program eligibility, conducts interviews, and completes assessments with expectant parents or parents who have a child less than three months of age. The Bilingual Case Manager is responsible for providing home-based prevention services focusing on child development education and coordination of services and activities for overburdened families of newborns. The Case Manager also conducts community outreach within the program’s target area to increase program enrollment as needed.
High school diploma or equivalent required.
NYS-AIMH Infant Mental Health Endorsement is a plus.
At least 6 months' experience in working with or providing services to children and families required.
Knowledge of infant and child development required.
Proficient computer skills, including Microsoft office, required.
Excellent verbal and written communication skills required.
An ability to establish trusting relationships and a willingness to engage in building reflective capacity, i.e., capacity for introspection, communicating awareness of self in relation to others, recognizing value of supervision, etc.
The nature of the position involves a fast-paced working environment with multitasking of many different duties and tasks. General daily priorities may change at a moment’s notice, and the position requires quick response time and flexibility.
Valid and clean NYS Driver’s License required.
Bilingual in Spanish required.
Office, community, and client home based work with frequent sitting, standing, writing and computer usage.
Must be able to climb stairs and move through a facility and a home.
Moderate reaching, bending, stooping kneeling or crouching.
The position includes interaction with children that could include picking up a child as heavy as 40 pounds.
