The Case Manager serves as a role model for young adults aged 17-21, helping them transition into the community. Responsibilities include developing transitional living plans and supporting residents in education, employment, and daily living skills.
Requires a Bachelor's degree in any field with two years of case management experience, or a human services degree with relevant experience. A Master's degree in a human services field with a qualifying student placement is also acceptable.
