The Wellness Coach empowers patients to engage in their own health through educational classes and group sessions focused on prevention and wellness. This role involves collaborating with care teams, documenting patient care in EMR systems, and utilizing motivational interviewing to support personal wellness goals.
Candidates must have a high school diploma, be bilingual in Spanish and English, and obtain Community Health Worker certification within six months. A bachelor's degree in a health-related field and coaching certifications are preferred.

The Wellness Coach provides support to both patients and the care team in helping to facilitate and empower patients to engage in their own health.
Utilizing the risk stratification tool for overall health status, this role is primarily working with patients served by every clinic that have scores ranging from healthy through low-rising risk with an emphasis on prevention-oriented work.
This role will be based at our offices within our Center for Learning and Innovation and the Wellness Coach will travel to our clinics as needed.
