Job summary

The Community Health Worker delivers the program model and will plan and implement culturally sensitive and effective in-home family education services, as directed by service recipients, applicable stakeholders and collaborative partners, while ensuring that services provided are in alignment with the Social Determinants of Health through a trauma informed care lens as defined by the agency. • Education/Training/Experience 1.
• High school diploma or GED certificate.
• 2.
• Completion of Community Health Worker certificate preferred, or experience in community-based environment.
• 3.
• Complete an approved Nurturing Curricula and be able to utilize various learning techniques in the successful delivery of the Nurturing Curricula.
• 4.
• At least two (2) years’ experience in the delivery of service those are resident-driven and work with families to address multiple issues in-home and/or community-based environment preferred.
• 5.
• Maintain certification in at least one approved educational curricula required by agency stakeholder.
• 6.
• Fluent in English and Spanish (read, write and speak) desirable.
• 7.
• Certificate or license, as applicable, by the appropriate State agency as applicable.
• 8.
• State of Texas vehicle operator's license and proof of liability insurance.
