What You’ll Do · Serve as a case manager for your assigned individuals, helping to guide their goals, supports, and daily needs · Coordinate services, appointments, and plans of care; ensure follow-through on all recommendations · Build strong relationships with families, guardians, and interdisciplinary team members · Support individuals in community activities, recreational programs, and daily living routines · Maintain clear and accurate documentation, including program data, notes, billing, and medical information

Requirements

What We’re Looking For · High school diploma or equivalent · Valid driver’s license and reliable vehicle, ability to possess a State of CT Public Service License · Advanced computer skills (Microsoft Office, data entry, billing · Attention to detail · Compassion, patience, flexibility, and sound judgment · Ability to lift up to 50 pounds
