  • Summary The Family Coordinator for the School-based Early Support Program plays a pivotal role in providing comprehensive support and intervention services to children and families within the school setting.
  • This position requires a compassionate and organized individual with a strong understanding of child development, family dynamics, and community resources.
  • The Family Coordinator will work closely with school staff, families, and community partners to assess needs, develop individualized plans, and coordinate services to ensure the well-being and academic success of children.
  • Essential Functions: Assessment and Planning: Conduct thorough assessments of children and families to identify strengths, needs, and goals.
  • Collaborate with school personnel and other stakeholders to gather relevant information and develop comprehensive support plans.
  • Utilize evidence-based practices and assessment tools to inform decision-making and goal-setting.
  • Service Coordination: Coordinate and facilitate access to a range of support services, including educational, mental health, medical, and social services.
  • Act as a liaison between the school, families, and external service providers to ensure seamless communication and coordination of care.
  • Monitor service delivery and progress towards goals, adjusting plans as needed to address evolving needs.
  • Advocacy and Empowerment: Advocate for the needs and rights of children and families within the school and larger community.
  • Empower families to navigate systems and access resources by providing information, education, and support.
  • Promote family engagement in decision-making processes and encourage self-advocacy and resilience.
